The following services run throughout our PCN, however please note that you will only be booked into or referred to them if deemed clinically appropriate and/or suitable.
Social Prescribing Link Worker Service
Social Prescribing Link Workers aim to mitigate health inequalities by assisting individuals in identifying and addressing complex issues impacting their well-being. They empower individuals to take charge of their lives, develop new skills, and engage in community groups. If you would like to be referred to this service, please ask your GP.
Paramedic Service
Our paramedic service is designed to provide prompt and comprehensive assessments to patients who are unable to leave their homes. If you’re housebound and require an evaluation of your health needs, you may be scheduled to see one of our paramedics.
Clinical Pharmacy Service
Clinical pharmacists possess a comprehensive understanding of medications and can provide valuable assistance to individuals in various ways. One such approach is conducting structured medication reviews for patients diagnosed with chronic health conditions. This process prioritizes patient safety, outcomes, and value while maintaining a person-centered approach. You may be booked in with a clinical pharmacist by our trained reception teams if you require support from them.
First contact physio service
Our in house physio is trained in evaluating and diagnosing muskoskeletal problems, offering proficient suggestions on the most efficient approaches to address their conditions, and referring them to specialized services, if deemed necessary. Our surgery staff may refer you to the first-contact physio or you can call and request to see them.
Extended access clinics.
We’re offering extended access hours to better serve our patients. We understand that busy schedules can sometimes make it challenging to book appointments during regular hours, so we’re extending our availability as follows:
Monday to Friday: 18:30 to 20:00
Saturday: 09:00 to 17:00
These extended access appointments will be conveniently held at Wood Bridge Medical Centre, located at Jubilee Gardens, Southall UB1 2TJ. This expansion of our service hours is aimed at providing you with more flexibility and convenience when seeking medical care. To Book an appointment, please call reception during opening hours.
